THIS NOTICE OUTLINES THE USE AND DISCLOSURE OF YOUR HEALTH AND MEDICAL INFORMATION, AS WELL AS YOUR ACCESS RIGHTS. PLEASE READ IT CAREFULLY.

This Notice of Privacy Practices applies to Juno Bio, except in the scope of clinical trial tests, or other activities that do not involve standard electronic transactions as defined by the Department of Health and Human Services ("HHS").

Your Privacy is Important to Us

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Who Will Follow This Notice

This notice applies to Juno Bio, its employees, and other personnel who provide services at our locations. All of these individuals will follow the terms of this notice when using or sharing your health information.

Our Responsibilities

We are required by law to:

  • Keep your health information private and secure.
  • Provide you with this notice of our legal duties and privacy practices.
  • Follow the terms of the notice currently in effect.

How We May Use and Disclose Your Health Information

We may use and share your health information for the following purposes:

1. Treatment

We may use your health information to provide you with medical treatment or services. We may share your information with doctors, nurses, and other healthcare professionals who are involved in your care.

2. Payment

We may use and share your health information to bill and collect payment from you, your insurance company, or a third party. For example, we may share information with your health insurance plan to obtain approval for a service.

3. Healthcare Operations

We may use and share your health information for health care operations, improve your care, and contact you when necessary. For example, we may use your health information to manage your treatment and services.

4. Appointment Reminders and Health-Related Benefits

We may contact you to remind you of your appointments, inform you about treatment options, or tell you about health-related benefits and services.

5. As Required by Law

We will share health information about you when required to do so by federal, state, or local law.

6. Public Health and Safety

We may share your health information for public health activities, such as preventing or controlling disease, injury, or disability. We may also share your information to report abuse, neglect, or domestic violence, or to prevent a serious threat to health or safety.

7. Research

Juno Bio may use or disclose your health information for research purposes under specific conditions, always prioritizing the protection of your data. We will only do so when one of these three conditions are met: we obtain your explicit authorization for the research activity; a special committee determines that there are adequate safeguards in place to protect your information and that the research poses minimal risk to your privacy; we ensure that only de-identified information is used, meaning that any data shared cannot be traced back to you personally.

8. Lawsuits and Legal Actions

We may share your health information in response to a court order, legal demand, or other lawful process.

9. Individuals Involved in Your Care or Payment for Your Care

We may share your health information with a family member, friend, or other person who is involved in your care or payment for your care, with your permission. For minors, we may disclose protected health information to parents or legal guardians as permitted by applicable federal and state laws, and regulations.

10. Business Associates

We may disclose your health information to our business associates who perform functions on our behalf or provide us with services if the information is necessary for such functions or services. Our business associates are required to protect your information and are not allowed to use or disclose any information other than as specified in the Business Associate Agreement.

11. Law Enforcement

We may disclose health information to law enforcement officials when required by federal, state, or local law. This includes responding to court orders, subpoenas, warrants, summons, or similar legal processes. Any such disclosure will be limited to the specific information requested and as permitted by applicable laws and regulations.

12. Public Health

We may disclose your health information for public health activities, including preventing or controlling disease, injury, or disability; reporting births and deaths; reporting child abuse or neglect; sharing information with the FDA about product safety, including reporting medication reactions or product issues, and notifying about product recalls; notifying someone who may have been exposed to a disease or may be at risk of getting or spreading a disease; in certain limited situations, informing employers about workplace-related illnesses or injuries.

13. Health Oversight Activities

As required by law, we may share health information with health oversight agencies for specific activities. These activities can include audits, investigations, inspections, and licensure processes, which are designed to monitor the healthcare system and ensure compliance with regulations.

14. Coroners, Medical Examiners, and Funeral Directors

We may disclose health information to a coroner, medical examiner, or funeral director when an individual dies, as authorized by law, to assist them in carrying out their duties.

15. Personal Representative

If you have a personal representative, such as a legal guardian, as established by applicable law, we will treat that person as if that person is you with respect to disclosures of your health information.

16. Correctional Institution

If requested by a correctional institution or law enforcement official, we may disclose health information necessary for your health, safety, and security, as well as that of other individuals within the institution.

17. Serious Threat to Health or Safety

We may use and disclose your health information when necessary to prevent or lessen a serious and imminent threat to your health and safety or the health and safety of the public or another person.

18. Government Functions

We may disclose your health information for specialized government functions when legally required, such as national security, military affairs, and protective services.

19. Workers' Compensation

We may disclose your health information as required to comply with workers' compensation laws and other similar legally-established programs that provide benefits for work-related injuries or illnesses.

20. De-identified Information and Limited Datasets

We may use and disclose health information that has been de-identified by removing certain identifiers making it unlikely that you could be identified. We also may disclose limited health information, as permitted under HIPAA regulations for limited data sets.

Your Rights Regarding Your Health Information

You have the following rights concerning your health information:

1. Right to Inspect and Copy

You have the right to inspect and obtain a copy of your health information, with certain exceptions. To access your health information, submit a written request to our Privacy Officer.

2. Right to Amend

If you believe your health information is incorrect or incomplete, you may ask us to amend it. Submit your request in writing, along with a reason that supports the amendment, to our Privacy Officer.

3. Right to an Accounting of Disclosures

You have the right to request a list (an "accounting") of certain disclosures we made of your health information. This does not include disclosures made for treatment, payment, healthcare operations, or those you authorised.

4. Right to Request Restrictions

You have the right to request limitations on how we use and disclose your health information for treatment, payment, and healthcare operations, or to individuals involved in your care. While we will consider your request, we are not obligated to agree unless it concerns a disclosure to a health plan for payment or operations (not treatment) that you've paid for out of pocket in full. If we agree to other restrictions, we will document them and follow them, except in emergencies when the disclosure is for treatment.

5. Right to Request Confidential Communications

You may ask us to contact you in a specific way (for example, at your home or office) or to send mail to a different address. We will accommodate reasonable requests.

6. Right to a Paper Copy of This Notice

You have the right to a paper copy of this notice at any time. You may also view or download it from our website.

7. Right to File a Complaint

If you believe your privacy rights have been violated, you have the right to file a complaint with us or directly with the U.S. Department of Health and Human Services (HHS). To file a complaint with us, please use the contact details below. We will not retaliate against you for filing a complaint. To file a complaint with HHS, visit www.hhs.gov.

Changes to This Notice

We reserve the right to change this notice at any time, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our website.

How to Exercise Your Rights

To exercise your rights, submit a written request to our Privacy Officer.
